- No Cleanouts (TMS 602 Art. 3.5 B.1.b / IBC 2104.3): Required for pours > 4 ft high (your 12-ft wall is 3x that limit). You cannot verify bar placement, lap splices, cell alignment, or remove mortar droppings/slurry from the bottom course.
- Improper Consolidation (TMS 602 Art. 3.5 B.2 / ACI 308R): A 2x4 is not a vibrator. It cannot consolidate 3/4" aggregate in a narrow cell around vertical bars. Expect massive honeycombing, voids, and zero bond to rebar.
- Cold Joint Risk (TMS 602 Art. 3.5 B.3): Pouring 12 ft in "one shot" from the top without cleanouts means the bottom 4–6 ft likely set before the head pressure could push grout down, or the mix segregated (aggregate hung up, paste ran down).
- Grout Spec Issues: 3/4" aggregate is too large for standard CMU cells (TMS 602 Table 2, Note 2: max agg = 1/3 cell width, typically 3/8" or 1/2" pea gravel). 4" slump is low for high-lift grout without vibration (should be 8–11" for self-consolidating or pumped high-lift).
What the Inspector Will Do:
- Check for cleanout openings (missing).
- Tap cells with a hammer (hollow sound = voids).
- Check rebar projection/alignment at top (likely displaced by 2x4 "shoving").
- Fail the wall.
Remediation (Expensive):
- Core drill every cell at bottom course to prove fill/bond (destructive testing).
- If voids found: Demo and rebuild. Grout cannot be "topped off" after initial set.
- If by miracle cores are solid: Engineer must stamp a variance letter accepting the non-standard method (unlikely for 12-ft lift).
